Dear Field Instructor

Thank you for your willingness to serve as a feild instructor for Brown School studetns. For each academic year in which you provided supervision for a Brown School student, you are eligible to receive three (3) continuing education units.   Three continuing education units for supervision as a field instructor is the maximum number of units allowed in a licensure period.

Please complete the form below to ensure that you receive a certificate verifying your participation as a field instructor for the George Warren Brown School of Social Work.

* State regulation requires us to validate your identity for issuing CEUs. In an effort to protect your privacy, we will no longer collect full Social Security Numbers.  Below you will be asked to provide your choice of either (1) the last four digits of your SSN or (2) your complete Social Work License number.
